let me now more precisely,
what does mean by this INT_ext flag bit 31 in datagram?
What are the possible cases when this flag bit is set?

This refers to the TMC428 interrupt. It comes via the SDO_428 pin, when TMC428 is not selected.

So, whenever TMC428 is not selected this INT_ext staus is set to 1 autonomously?
Does INT_ext status get affected only by this event of not recognizing 428 via SDO428?

No, the TMC428 has an interrupt output, signaled to its SDO pin. This interrupt flag is mirrored to the TMC423 datagram.
